Aim: The aim of this article is to determine whether tolerance developed to the effects of tadalafil on erectile function (EF) over a 6-month treatment period.
Methods and main outcome measures: Post hoc analysis of data from a multicenter, double-blind, randomized, placebo-controlled, parallel group study was performed. Men (> or =18 years of age) with erectile dysfunction (ED) were randomized to treatment with placebo (N = 47) or 20-mg tadalafil (N = 93) taken as needed for 6 months. This report focuses on efficacy assessed with the Sexual Encounter Profile (SEP) diary (diaries were collected after a 4-week treatment-free run-in period [baseline], and monthly for 6 months), and with the International Index of Erectile Function (IIEF) (administered at baseline, and at 3 and 6 months). RESULTS. The mean per-patient percentage “yes” response on SEP question 3 (SEP3, successful intercourse) was 33 +/- 4% at baseline, 74 +/- 4% after 1 month, and 78 +/- 4% after 6 months of tadalafil treatment. The IIEF EF domain score was 16.2 +/- 0.7 at baseline, 24.3 +/- 0.8 after 3 months, and 24.3 +/- 0.9 after 6 months of tadalafil treatment. In a subgroup of patients who took tadalafil > or =3 times per week (N = 24), the SEP3 score was 87 +/- 4% after 1 month and 93 +/- 3% after 6 months of treatment, and the IIEF EF domain score was 27.3 +/- 0.9 after 3 months and 28.5 +/- 0.4 after 6 months. Of 16 tadalafil-treated patients who discontinued, three cited a lack of efficacy.
Conclusions: Tadalafil treatment significantly improved SEP3 and IIEF EF domain scores. The efficacy of tadalafil, taken as needed, was maintained over a 6-month treatment period in men with ED.

Actually, I’m trying something new. Based on that video that was presented on skin aging using testosterone in a cream. Not for TRT for face wrinkles and plumping.
I have some extra testosterone my TRT prescription. And, so I used one ml 200 mg testosterone with about 2 ml of DMSO, and then used 10 squirts of Palmer’s face/body lotion because it already has oil and wax, and is very quick absorbing. The testosterone/cypionate is in oil already. Just made my own little skunk works product… for face wrinkle lotion. It blended very easily. Putting on tonight. Let’s see if my results in 6 months are as good as those nursing home subjects in 1967. Made enough for probably a month or more. Will use nightly and in the morning.

Here’s the update. Woke up this morning after putting this new TRT lotion on my face and surprisingly, everything looked pretty good this morning. No reaction, no swelling, no redness. Skin felt soft… actually was less puffy in the under eyes… showered and put on another dose of lotion.
Let’s see how this goes the next week…

Having done many of these in the ER … it’s not just the pain, it is often permanent loss of function after treatment.
Briefly the process is a nerve block, then a 19 or 21 ga butterfly needle into each into each corpus carvernosum with irrigation and removal of blood and injection of phenylephrine. Over 50% of the cases I’ve seen are not related to PDE-5’s.
However, this is rare enough - I’m happy enough taking 5 mg a day.

I think you need to put this in a separate thread. I am curious about this myself… maybe testing it on my neck or the top of my forearm where I have sun damage.

Will do… these aren’t tadalafil in any way.

I’d imagine a Trimix injection is the most common cause of priapism and the only solution to the permanent loss of function is a penile implant, or “the button” as I once heard somebody call it.

Interestingly, and it is a small group of folks I’ve treated - although I had 1 patient each, 2 days in a row a couple of years ago. Trazodone has been the more common culprit in my small group.

Why not Hydroxyzine with shorter half life? Do you use doxylamine long term?
