I’m wondering if it would be safe to potentially add low dose Tadalafil if one is already low in weight and has low blood pressure, as it may further decrease blood pressure?

Be careful with tadalafil. If you have pain in your legs at night, stop taking it. I am one of those people that have had this side effect. In my case it was severe. I don’t have the same reaction to sildenafil. Go figure.

The beauty is you can test, try 1.25 mg, then 2.5 mg, then 5 mg … I think 5 mg/day for longevity/neurocognitive decline is a reasonable final dose. You can monitor your BP and make sure you aren’t getting into trouble. I agree with @LukeMV for most people only little effects in this area - but If your SBP is currently 86 … then yes, this could be a problem, if it is 110 mmHg, then less of a worry.

Thank you, Dr. Fraser. That’s a great suggestion, to slowly try to increase doses. […] “for most people only little effects in this area - but If your SBP is currently 86 … then yes, this could be a problem.” It is around 90/60 and has been so for years. (Only if I drop further in weight I may experience a further decrease). So I may really want to go slow and monitor my BP, that’s a good suggestion.

There is another important consideration to this drug choice. Almost 1/3 of older american men have an enlarged prostate. Frequent urination is a bothersome symptom that can also become a sleep issue.

Fortunately, we have two different drug treatments (Alfuzosin and Dutasteride) that are quite effective. With the quarded approval of my Primary Physician I take both treatments. I later read through my own research that low dose Cialis and Viagra can also reduce urinary frequency. I got my doc to sign off on that one as well. We do semi annual blood work at minimum and so far so good. It may make more sense to look at Cialis because of the much longer half life. The longer it lasts, the less you pee! This can specifically be timed to improve sleep with less interruptions.

BTW I have been on Rapamycin for about 12 months. One a week at 6 mg. I didn’t see any effect on my BPH but did see some immediate improvement in gum health with reduced gum bleeding.

I can’t point to many specific Rapa effects, but I have noticed almost no negative effects when recieving typical vaccinations and the worst cold/flu thing I went through since Rapa lasted one day of mostly sleep and was back to work the next day. Less illness in general. With Rapa we are playing the long game.

I work out 6 days a week, 3 days cardio, 3 days resistance. I eat Keto and practice intermittent fasting.

I tried taking saw palmetto for my BPH and it had very little effect. I was getting up to pee 4 to 5 times a night. I trained myself to basically sleepwalk my way through it. But now with my 3 drug BPH treatment it is 1 to 2 times a night. The health benefits of that are huge.

I get back pain every time I take it. It’s not an uncommon side effect. As far as I know, the mechanism is unexplained. I take 50 mg a day of sildenafil instead. My face turns a little red but tolerable.

I am wondering about the specifics of timing to maximize the reduction in the need to pee at night. Is there a recommended time to take Tadalafil for this effect? I’m thinking to maximize the benefit maybe taking it around dinnertime? Or possibly bedtime? I try not to take anything around bedtime, because I usually need to drink water to get the pill down, but drinking water at bedtime would be counterproductive for me.

It would seem that 1 to 2 hours before bed would maximize the effect (or perhaps right at bedtime)… here is the blood level graph for Cialis:

I think to get the maximum effect I take it around 30 min before sleep time. Another interesting option is to ejaculate before sleep as well. There is proven science to that. Old school, clearing the pipes:)

Could also make the side effects more tolerable perhaps as long as it doesn’t cause insomnia or anything.

I may switch to this from my usual breakfast dosing

So with taking it every 24 hours, remember, this will boost the level on top of what already it there. We generally say that you don’t have your final level and stability until ~5 half lives worth of dosing is completed - possibly a little more - so in this case really until you have 4 to 5 daily doses stacked, you’ll then have more substantial levels over the 24 hours - not that it won’t be dropping still - just not to as low as this shows it at 24 hours.
