There is another important consideration to this drug choice. Almost 1/3 of older american men have an enlarged prostate. Frequent urination is a bothersome symptom that can also become a sleep issue.
Fortunately, we have two different drug treatments (Alfuzosin and Dutasteride) that are quite effective. With the quarded approval of my Primary Physician I take both treatments. I later read through my own research that low dose Cialis and Viagra can also reduce urinary frequency. I got my doc to sign off on that one as well. We do semi annual blood work at minimum and so far so good. It may make more sense to look at Cialis because of the much longer half life. The longer it lasts, the less you pee! This can specifically be timed to improve sleep with less interruptions.

Welcome to the forums!
You are right - here is a paper on this benefit:
This integrated analysis revealed a small but statistically significant median maximum urinary flow rate improvement for tadalafil vs placebo.

BTW I have been on Rapamycin for about 12 months. One a week at 6 mg. I didn’t see any effect on my BPH but did see some immediate improvement in gum health with reduced gum bleeding.
I can’t point to many specific Rapa effects, but I have noticed almost no negative effects when recieving typical vaccinations and the worst cold/flu thing I went through since Rapa lasted one day of mostly sleep and was back to work the next day. Less illness in general. With Rapa we are playing the long game.
I work out 6 days a week, 3 days cardio, 3 days resistance. I eat Keto and practice intermittent fasting.
I tried taking saw palmetto for my BPH and it had very little effect. I was getting up to pee 4 to 5 times a night. I trained myself to basically sleepwalk my way through it. But now with my 3 drug BPH treatment it is 1 to 2 times a night. The health benefits of that are huge.

My Dr prescribes, and I fill through Amazon pharmacy. 5mg X 30 delivered for $13.
I have been very happy with the results. Also, the health and longevity benefits look promising.

I will have to switch to 3 month when I need a new prescription and save a few dollars.

I get back pain every time I take it. It’s not an uncommon side effect. As far as I know, the mechanism is unexplained. I take 50 mg a day of sildenafil instead. My face turns a little red but tolerable.

I am wondering about the specifics of timing to maximize the reduction in the need to pee at night. Is there a recommended time to take Tadalafil for this effect? I’m thinking to maximize the benefit maybe taking it around dinnertime? Or possibly bedtime? I try not to take anything around bedtime, because I usually need to drink water to get the pill down, but drinking water at bedtime would be counterproductive for me.
It would seem that 1 to 2 hours before bed would maximize the effect (or perhaps right at bedtime)… here is the blood level graph for Cialis:

I think to get the maximum effect I take it around 30 min before sleep time. Another interesting option is to ejaculate before sleep as well. There is proven science to that. Old school, clearing the pipes:)

Could also make the side effects more tolerable perhaps as long as it doesn’t cause insomnia or anything.
I may switch to this from my usual breakfast dosing

So with taking it every 24 hours, remember, this will boost the level on top of what already it there. We generally say that you don’t have your final level and stability until ~5 half lives worth of dosing is completed - possibly a little more - so in this case really until you have 4 to 5 daily doses stacked, you’ll then have more substantial levels over the 24 hours - not that it won’t be dropping still - just not to as low as this shows it at 24 hours.

It appears that you never received any answer so today 5/30
GoodRx = 90 x 20mg = $38.35 so $0.43/20mg or eleven cents if you take 5mg (cut x four)
PharmacyChecker.com Offshore Cheap Meds 90 x20mg = $0.37/20mg tablet so ten cents per 5mg dose.
I have personally used Offshore Cheap meds many times both for Cialis and other meds. They are a legit Canadian Pharmacy that ships from wherever… Singapore, Germany…India.

Unless you try to use amyl nitrate or someone thinks you’re having a heart attack and uses sublingual nitroglycerine, in which case one can experience a serious (fatal?) drop in BP.

My doc had me agree to no more than one every two days, I’m 74. Cialias 20mg tabs.
I’m apt to use 5mg (1/4 of a 20mg) or 1/3 tablet. I call sildenafil and tadalafil the 3-step med. Snort a 12-hour decongestant, take some acetaminophen for the headache and some antacid for indigestion. YMMV.
Interestingly I took quite a bit more (20mg of Cialis, or 20mg of Sildenafil) a decade + ago and over the years the dosage that I’ve needed for ED has dropped radically from 20 >10> 1/3 tab > 1/4 tab to really not necessary these days. 12 years ago my lipid profile was awful, today “brilliant.” I consider the “penimeter” the indicator of health of a man’s circulatory system and over 12 years I’ve seen enormous improvement.
I just came across the concept as using it for longevity and landed here. What dosages are considered good for that? With the side-effects I can’t imagine taking, everyone agree w/ Dr. Fraser’s post above?
Btw…it’s never affected my BP noticeably but nothing really seems to affect that… Losing 54lbs, aerobics… strange (but I’m old…74) I’d love to go off one of the three BP meds I’m on.
BTW, why buy 5mg when you can chop a pill into half/quarters, or pulverize it and even come up w/ 2.5mg. OffshoreCheapMeds from Pharmacychecker.com at $0.37 / 8 = 5 cents a dose for 2.5mg. It’s neither critical dose nor is it coated as far as I know.

Is it a cramp-like pain that you get? That’s what I get in a leg, occasionally a foot. Severe pain waking me up, I have to walk before the cramp goes away. This has been 2-3 nights a week the last month or two, been on tadalafil for 6-8 months.

Three clinical trials found that oral administration of tadalafil, a PDE5 inhibitor, reduced MDSCs and restored antitumor T cell immunity in patients with head and neck squamous cell carcinoma [14,15] and metastatic melanoma [16].Oct 10, 2018
No Causal Link between Phosphodiesterase Type 5 Inhibition and Melanoma
I wonder if someone read this, a bit too quickly and came to an odd conclusion about it causing melanoma:
Tadalafil has biologic activity in human melanoma. Results of a pilot trial with Tadalafil in patients with metastatic Melanoma (TaMe)
I know plenty of folk who might look at that and say: “OMG, Tadalafilr causes metastatic Melanoma.”
The reality is quite otherwise and the take-away is:
For those with metastatic Melanama (this article) says it’s ok to use Tadalafil.
