This site is an amazing resource! I searched and I haven’t found much detail about lipid changes. My apologies if I’ve missed discussions about this issue.

I am a 70 year old male and I’ve titrated up to taking rapamycin 4mg weekly for healthspan and longevity. After 6 weeks my Apolipoprotein B increased from 84 to 92, and LDL increased as well. Why is this happening? What should I do about it? I’m already on 40mg Crestor and Zetia. Would it make sense to switch from ezetimibe to Nexlizet?

On the bright side My “C-Reactive Protein, Cardiac” decreased from 0.40 to 0.29.

Do lipids settle down after a while or is this a permanent elevation as long as I take Rapa?

How have others handled this?

Likely by increasing PCSK9. I would lower that apoB and LDL further. Bempedoic can help some but a PCSK9 inhibitor would probably decrease it by ~50%.

My LDL was 67 before I started Rapamycin. After I started, it went up to 120. I added Bempedoic Acid and Ezetemibe and it came back down to 67. I’ve since added 5 mg Atorvastatin and my LDL and ApoB are now 48.

My recent blood work came back with my LDL up to 163 mg/dL and my ApoB at121 mg/dL! All other relevant markers were and have been consistently very good to excellent. Unfortunately. it’s my first time testing ApoB. 2 1/2 yrs, weekly taking Sirolimus (Dr. Green patient :disappointed_relieved:), but recently upped to 6 mg from 4 mg. 68 y/o female, 115-120 lbs on a multitude of supplements and a very clean diet. I’m going to take a break from Sirolimus for a few months and see if that impacts my outcomes. Doc ordered a CAC just to be safe. I’d appreciate any and all opinions, ideas, criticisms (go easy on me lol), thoughts, conjectures, etc. Will retest at the 3 month mark to see if break from Sirolimus had an impact.

Do you have a family history of lipid elevations? I do, testing with triglycerides around 400 in my 20s. This was before statins, but I’ve been on something or other ever since. Might rapamycin elevate blood lipids especially in people with familial hyperlipidemia?
