Hey Daph… I test my blood for rapamycin every 3 - 4 months using LABCORP at my doctor’s office.
From the many LABCORP tests over the past 3-years… with and without Grapefruit juice (GFJ), I know straight 6 mg rapamycin with nothing… gives me 6 mg. If I add a fresh squeezed Red Grapefruit… about 5 fl.ounces with my dose… it magnifies the rapamycin in me 3.5 to 6 times.
When I take 1 pill of 2 mg rapamycin with GFJ … my LABCORP test taken 2 hours later says I have 8.6 ng/mL in my blood. My c-max. About a 4 'xs increase… based on test results.
So my goal is always 6 mg in me at my weekly dose… not concerned if it is a few points higher. But do not want to have more than 12 mg. Based on biological testing… my age reduction sweet spot in dose is 6 mg.

I don’t know if you are aware, but Ulta Lab now offers a rapamycin test.
I don’t know how long they have been offering it, because I used the wrong search term rapamycin instead of sirolimus.
Perhaps more convenient than dealing with Life Extension. They also have one for Tacrolimus.
They are not the cheapest available, I think Marek is the cheapest, but it might be convenient for some.

I have done quality assurance tours of both Biocon and Dr Reddy in India as part of my previous employment and can assure you they are real companies who are regulated by international regulatory agencies like the FDA. Their quality control and quality assurance processes are similar to those of the big pharma companies based in the US and Europe.

@Agetron So, 6 - 12 mg works for you, and 36 mg is too much. Have you tried something halfway? Maybe 24 mg? Since we don’t see any MTOR rebound at below 20 mg, and we can see it at 40 mg, I’m wondering if a dose around 20 mg would be a sweet spot?
For me, I’m currently doing 14 mg (4 mg + GFJ) and 10 mg ( 3 mg + GFJ) on alternating weeks. I think my body has gotten used to it as the euphoria I used to experience is waning.
I’m always on the lookout for that sweet spot for longevity.

Hey Chris… after getting great biological marker results at 6mg… and slowly worsening… negative results at 36mg.
I really have no interest in trying higher than 12 mg… which might still be too high… I will do another GlycanAge inflammation test and TruMe methylation test in April… See what the results say. Look at the 3 + years linear results.

I have not. But the link you provide leaves the impression that most of the issues are in the past, and it’s not cosistent with what I’ve seen in the last year. Suprise inspections do happen and facilities are spotless, same as they are in Europe and the US. The hygiene is especially impressive given how filthy Indian cities are generally.

Recently switched suppliers to Welcome Wellness. Price was only $1/mg and they accept payment with CashApp. Was uncertain if Biocon was legit so used Marak to test
10 mg after 1.5 hours yielded a result of 46.8

Basically you’re saying that testing after 24 hours is a bad idea. I took 6mg with GFJ and after 24.5 hours it is sitting on 3.2ug/L.
Should I boost it to 10mg? Should I test it after 3 hours or maintain 24 hours?
GFJ 1 or 2 hours before? I’ll do another test next week.
GFJ 1-4 hours before should yield similar effects.
Just don’t let it go over 241 minutes. Then you’re screwed. 
Actually the effect does fall off a cliff somewhere between 4-5 hours as the enzyme levels recover.

In the above study where the peak sirolimus levels were measured, is it known whether those people were taking sirolimus with or without food?
I just did my first trough and peak tests immediately before and 2 hours after a 6 mg dose with a light meal and fish oil, and the levels were 0.06 ng/mL and 28.8, respectively. That dose equates to 2.3 mg/m2 for my height and weight. I think that’s pretty consistent with the results in this study, maybe a tiny bit higher?

They took the dose on an empty stomach after fasting overnight and for 4 hours afterwards. I was surprised it was administered as a nonaqueous solution diluted with distilled water. I thought it had to be in an enteric coated pill?

It may have been this:
There is a special formulation of sirolimus for (I assume) people who can’t swallow pills. I’m sure its bioavailable, as its made by Pfizer (under the “Rapamune” brand, and its used for organ transplant patients. Its not used too often I suspect - as most people can take pills/tablets. With GFJ could mean a lot of different things. How much GFJ did you take with it?
My suggestion would be to test at 24 hours without GFJ for comparison to see the difference. If the difference is small then maybe the dose of GFJ was a bit too low.

Regardless I think my level is too low. Next week I’ll try 12mg with GFJ and the following week I’ll do 12mg without GFJ to compare. I’m getting annoyed that I can’t boost the level. I’ll like to see what I need to get closer to 30ng after 90 minutes. If without GFJ is similar I’ll forgo it from that point onwards.
Consider trying 6 mg with a larger amount of GFJ. The amount can make a huge difference in terms of what kind of boost you get in blood concentrations.

I’m on my last 250ml of GFJ lol. I buy a 2L bottle and freezes them in 250ml portions to take every week. To be honest I don’t like GFJ lol. Ok. This week I’ll take it for the last time with 6mg and I’ll check my blood at 90mins and day 4 like Bryan Johnson just so now I’ve all my data at 6mg with 2hrs, 24hrs and 7 days. After that I’ll try 500ml with GFJ. But I’m thinking of testing at 90min and 4 days intervals. What do you think?

Interestingly my 24 hour result is similar to Bryan Johnson when he took 6mg.
Hell. I’m tired of the low levels considering my body mass. Next week I’ll start with 12mg with 250ml GFC.
