I think this article is more focused on the supplement side of things, and not on drugs like rapamycin. In rapamycin the link to autophagy is much more clear and robust - see research at bottom of this post.

The biological clean-ups that could combat age-related disease

Could targeting autophagy — often likened to a cellular trash management system — extend life? Some researchers are unconvinced.

When it comes to drugs with broad autophagy-inducing potential, several medicines already being widely used seem to fit the bill. For example, metformin, a common diabetes treatment, and the immunosuppressant rapamycin, which is used to prevent transplant rejection, both fire up autophagy signalling through their effects on mTOR, a master regulator of several steps on the autophagy pathway.

In worms and mice, these drugs increase lifespan and overall well-being. But because they have many molecular effects, “you can never be sure what is due to autophagy,” says Beat Nyfeler, a chemical biologist at the Novartis Institutes for BioMedical Research in Basel, Switzerland.

Unfortunately, the molecular promiscuity of mTOR signalling, which also controls protein synthesis, and the nonspecific nature of the drugs can also result in unwanted side effects. “I think to really address the power of activating autophagy one would need to find something mTOR-independent,” Nyfeler says.

By repurposing an existing drug, David Rubinsztein, a cellular neurobiologist at the University of Cambridge, UK, has identified a blood-pressure medication that induces autophagy through other means. Called felodipine, it works by blocking calcium channels, both to relax blood vessels — hence its use to treat hypertension — and to increase autophagic activity.

In mouse models of Parkinson’s and Huntington’s disease, Rubinsztein and his colleagues have shown that the drug promotes the clearance of aggregate-prone proteins from the brain, resulting in behavioural and functional improvements9. A clinical trial is now planned to evaluate whether felodipine treatment can reduce levels of mutant huntingtin protein in the spinal fluid of people with early-stage Huntington’s disease.

Other companies, meanwhile, are looking to develop selective autophagy-activating agents directed at different therapeutic targets. At Samsara Therapeutics, scientists have approached the problem by exposing cultured cells to tens of thousands of drug compounds and then running assays designed to detect any changes in autophagy function. This work has yielded three promising drug leads, Hamley says, which the team is now trying to validate.

Tommy Wood says in this episode (-18:00) “…if you squint you see about the same autophagy in humans after 30 minutes of aerobic exercise as you see in 3 days of fasting.”

Anyone heard this before?

The study Tim refers to is marked as ‘in progress’ on the SA Health and Medical Research Institute website. This will be one to keep an eye on

This is really interesting… we definitely need more information on this (more research). I suspect a dose of 3 to 6 mg of rapamycin provides that same autophagy benefit, but it needs to be validated.

Also - I wonder what the exact relationship is, or correlation between mTORC1 inhibition and autophagy. @Krister_Kauppi mentioned that the Buck Institute is going to be looking at dose dependent mTORC1 inhibition via rapamycin in their next study, it would be nice if we could also map that to autophagy levels:

@RapAdmin Dr Tommy Woods is very well known in the athletic performance space. I first encountered him on the Nourish Balance Thrive podcast 5-6 years ago where he was a regular guest. He has a lab focusing on brain health at the University of Washington. His expertise covers a lot of ground : brain health, metabolism, physical activity, and human performance.

I don’t know if Alan Green’s patients participated in this, but if @DrFraser is picking up some people who have been on rapa a long time, maybe he can enroll in the ‘early access’ program and get them tested.

I posted my results elsewhere which reflect eighteen months of rapa use (at levels higher than the usual 6mg/week), and they show a very high level of ‘autophagy inhibition’ whatever that is. Which doesn’t sound good. Querying folks at Optispan (who submitted the test) has left me feeling less than enlightened, and Ms. Mitin, who I queried here, ain’t talkin’.

No idea what the test costs.

It would be interesting to see the results for some rapa.news participants.

That’s almost certainly not true. Also it’s difficult to compare the two. In what tissue are they talking about? Which measure of autophagy? I think exercise will mostly effect the muscles that were exercised, acutely at least, while fasting will tend to have a more of a full body autophagy effect.

Here is more detail on Dr Woods thinking (review of research?) on exercise vs fasting for autophagy. Exercise increases autophagy independent of nutrient status. I’m still looking for the study or studies that Dr Woods seems to be referring to.

“36 hours of fasting then exercise does not cause a higher rate of autophagy during the exercise.”
